Description of Service
The Alfred @ Home program is a hospital in the home program, which aims to provide individualised and holistic care to patients who require home based acute nursing care.
Alfred @ Home allows patients to receive ongoing treatment in the familiar surroundings of their own home.
Each patient is individually assessed to determine his or her needs and a plan of care is coordinated in consultation with a multidisciplinary team.
Alfred @ Home aims to:

Provide continuous patient care from the hospital to the home.

Minimise the disruption to the patient's regular routine during their treatment and empower the patient and their family.
