Description of Service

The centre-based program provides clients with short-term, intensive, centre-based rehabilitation for a usual period of 6-8 weeks.

The program assists clients who are having difficulty performing their daily activities following an accident, injury or medical condition.

The centre-based team includes:

  • Physiotherapy
  • Occupational Therapy
  • Speech Pathology
  • Social Work
  • Nursing
  • Allied Health Assistant
  • Aged Care Registrar available
  • Continence Service available

Group and individual therapy is offered. Groups are run by Physiotherapists, Occupational Therapists, Speech Pathologists and Allied Health Assistants and target:

  • Strength
  • Balance
  • Mobility
  • Communication
  • Hydrotherapy
  • Upper Limb Function
  • Public Transport Access
  • Falls Prevention
  • Independence with daily tasks
  • Integration with other activities and services

Fees

$7.80 for Individual sessions

$3.00 for group sessions

Eligible clients will not be denied a service because of an inability to pay.

Transport

Clients are requested to organise their own transport. However, if you are unable to provide your own transport options may be arranged.

Eligibility

People aged 18 years and over who:

  • Reside in the following council areas: City of Port Phillip and Parts of the City of Stonnington and parts of the City of Melbourne
  • Are medically stable and have a local GP responsible for their medical care.
  • Have had a recent illness, injury, or medical event resulting in a changed level of functioning.
  • Must have identified rehabilitation goals that are best achieved in a centre-based facility and may benefit from interdisciplinary input.
  • Are able to actively participate in a rehabilitation program.
  • Are willing and safe to travel into the centre via car/taxi and able to mobilise a minimum of 50m. (this includes wheelchair/scooter users).
  • Agree to participate.
